# Tasks

You may be assigned tasks from Prava for various administrative responsibilities. You can view all your open tasks at <https://app.pravatherapy.com/dashboard>. Below you can find a list of task types along with detailed explanations and instructions on how to resolve:

## Claims Validation Tasks <a href="#claims-validation-tasks" id="claims-validation-tasks"></a>

Claims validation tasks are any issues that occur during creation and sending of the claims to insurance. Resolving these in a timely manner is important to ensure that we are able to send the claim for reimbursement (and hence get reimbursed and be able to add to your payout).

On a validation error, the chart note will be unlocked to allow for fixes. The chart note will be unlocked even if it’s only data in the client’s profile that needs to be fixed. Always **remember to re-lock the chart note** after fixing the issue.

### Duplicate CPT Code Found <a href="#duplicate-cpt-code-found" id="duplicate-cpt-code-found"></a>

The same CPT code was entered twice in the billing section of the chart note.

To resolve edit the coding section to remove the duplicate.

### Calculated 0 units for CPT Code <a href="#calculated-0-units-for-cpt-code" id="calculated-0-units-for-cpt-code"></a>

When converting the minutes associated with a CPT code to a number of units (which is the value that insurance reimburses on) - 0 units was calculated. This is commonly the result of a typo in the Minutes field (e.g. entering a 2 instead of 20) or a nuance in interpreting the Medicare 8 Minute Rule. Please review [AMA vs. CMS Billing](/compliance-insurance-billing.md#ama-vs-cms-billing) if you are unsure how to fix or reach out to <provider.support@pravatherapy.com> and we can assist.

To resolve edit the Minutes field associated with the cpt code to the correct value or remove the code entirely if it’s not billable.

## Incomplete Chart Notes Tasks <a href="#incomplete-chart-notes-tasks" id="incomplete-chart-notes-tasks"></a>

In order to help ensure you don’t forget to submit a chart note for a completed session we monitor all your appointments and unsubmitted chart notes and send you reminders if something is missing. This does require keeping a clean calendar and marking appointments as cancelled as well as cleaning up obsolete chart notes, but saves a lot of headache down the line in ensuring your payouts accurately reflect the sessions you’ve had.

### Missing Chart Note for Appointment <a href="#missing-chart-note-for-appointment" id="missing-chart-note-for-appointment"></a>

This task will be created if you have an appointment on your calendar, but no associated chart note for the same client on the same day. Note that this feature requires you to enter your appointments on the Prava Calendar directly (syncing client appointments via Google Calendar is not sufficient).

To resolve:

* Create & submit the missing chart note, or
* Mark the appointment as cancelled

### Unsubmitted Chart Note <a href="#unsubmitted-chart-note" id="unsubmitted-chart-note"></a>

![Unsubmitted Chart Note screenshot](https://3656044907-files.gitbook.io/~/files/v0/b/gitbook-x-prod.appspot.com/o/spaces%2FF6YvmN64xqPwHOm4KMzh%2Fuploads%2Fgit-blob-c45ce4a4154c8e395ff84ffdbe61cf037ab9a752%2Ftasks-01.png?alt=media)

This task will be created if you have an incomplete (i.e. it hasn’t been locked) chart note for a session that occurred more than 24 hours ago.

To resolve

* Sign & lock the chart note to submit it, or
* Delete the chart note if it’s obsolete

### Reviewing assistant notes <a href="#reviewing-assistant-notes" id="reviewing-assistant-notes"></a>

![Reviewing assistant notes screenshot](https://3656044907-files.gitbook.io/~/files/v0/b/gitbook-x-prod.appspot.com/o/spaces%2FF6YvmN64xqPwHOm4KMzh%2Fuploads%2Fgit-blob-eb4ed5d862e6173578ef9d640152ccbb807d883a%2Ftasks-02.png?alt=media)

This task will be created once an assistant completed their session note and you are the lead PT/OT on the case (i.e. the one who submitted the Eval).

To resolve:

* Review the note and “Sign & Lock” it
* If you have any modifications you’d like to make you can Unlock the note and either modify directly or message the other provider to fix

## Required Documentation Reminder Tasks <a href="#required-documentation-reminder-tasks" id="required-documentation-reminder-tasks"></a>

In order to ensure we are staying in compliance with all government, state and payer requirements we will periodically require you to complete additional documentation for the client’s care.

### Progress Note Reminders <a href="#progress-note-reminders" id="progress-note-reminders"></a>

We require a progress note to be submitted every 10 visits or 30 calendar days (whichever comes first) for all commercial and government payers. Cash pay clients are excluded from this. After the 9th visit or after the 27th day since the start of the care plan - we will assign a task to remind you to complete your progress note.

To resolve, [complete a progress note](/charting.md#progress-notes) for this client before or during the next session.

### Recertification Reminders <a href="#recertification-reminders" id="recertification-reminders"></a>

As the care plan that was documented in the initial evaluation is expiring (either the number of weeks and total sessions have run out OR a maximum of 90 calendar days has passed), we need to complete a recertification note to get an updated care plan signed off by the referring physician. A task will be created 1 week before the care plan end date or once the total occurred visits are within 3 of the total care plan visits (e.g. if a care plan asks for 30 visits, we will assign a task after the 27th).

To resolve:

* [Complete a recertification note](/charting.md#recertification-notes) for this client before the care plan expires, or
* [Complete a discharge note](/charting.md#discharge-notes) for this client

## Querying This Documentation
If you need additional information that is not directly available in this page, you can query the documentation by asking a question.

Perform an HTTP GET request on the following URL with the `ask` and `goal` query parameters:

```
GET https://docs.pravatherapy.com/tasks.md?ask=<question>&goal=<user_goal>
```

`ask` is the immediate question: it should be specific, self-contained, and written in natural language.
`goal` is what the user is ultimately trying to achieve, the reason they need the answer. Sharing it helps GitBook give you a better, more relevant answer. A goal is most helpful when it describes the outcome the user wants rather than restating the question. For example, with `ask=how do I create an API token`, a goal like `automate deployments from our CI pipeline` lets GitBook tailor the answer to that use case.

The response will contain a direct answer to the question and relevant excerpts and sources from the documentation.

Use this mechanism when the answer is not explicitly present in the current page, you need clarification or additional context, or you want to retrieve related documentation sections.
